The team is made up of nurses and youth worker and work across South Staffordshire, currently offering support Monday to Friday, between 9am – to 5pm but are flexible to support outside these core hours.
We support young people between the ages of 8 years to 18 years with a range of mental health difficulties ranging from anxiety to self- harming behaviours to low mood.
We currently do not accept self-referrals, a CAMHS clinician will refer to Outreach and we will work alongside that clinician.
We offer home based interventions, working with young people and their families and can offer daily support (Mon – Friday) depending on their needs.
We have a flexible approach and can support young people at home, in school or somewhere in the community where they feel most comfortable.
We aim to prevent hospital admission but if needed, we support a quick and smooth transition back into the community.
We will work alongside schools and colleges to make sure young people have the right support, at the right time and in the right place.
