We need your help!
The NHS Youth Forum is working hard to improve healthcare transition within the NHS.
This survey for children and young people, parents and carers to complete as part of the NHS Youth Forum and Barnardo’s collaboration.
This has been put together by 6 young people to build a knowledge base on the current landscape of transition to adulthood from a young person’s perspective.
It will help inform them for a review of that data and development of resources for national use.
