As you approach your 18th birthday, your time using Child and Adolescent Mental Health Service CAMHS services will come to an end and we know there can be a lot of feelings around this time.
To help you move from CAMHS into Adult Mental Health Services (AMHS), you will be supported by the Transitions workers. They will work with you alongside your Lead Professional and may also allocate you a named Care Navigator. Your Care Navigator will work with you and your family for a short time from when you are 17 and a half to get to know you and support your move to other mental health services who can offer you the right support.
They will work with your consultant and/or lead person at CAMHS to ensure you have a smooth and safe transition to help support you in moving on from CAMHS across to adult services. Your Care Navigator can also help you with other areas that may be worrying for you, for example, they can help you by showing you how to access things like support services such as Vitaminds, Turning Point, social activities, football/music clubs etc or other things you might be interested in having a go at. They will help guide you to find ways you can get to them, where to go to get advice around finding work etc. They can also speak with your college to share information about things that support you as well as things you find tricky.
Everyone is different so the Care Navigator will work with you to create a plan that is made just for you and this will include all the information you need to be able to access what is on the plan. Â Should you be accepted into the Adult Mental Health team, they can attend some of the first appointments with you to introduce you to the new clinicians you will be working with. For anyone with the diagnosis of ADHD requiring transition to Adult ADHD service, the Care Navigator can support you when attending the ADHD transitions clinics.
We aim to prepare you for a smooth and supported move from CAMHS into Adult Services and can answer any worries you or your parent/carer may have about how other services might work differently to what you have already been receiving.
